This is only here as a convenient way to get awards reported so that they can go into the OP in a timely fashion. Please include as much relevant information as possible, including a return email address for me to use to contact you with any necessary questions or clarifications. Thanks.

Note: the "reference" name is important - it allows me to track down the correct identities of 'new' people, or mis-spelled names, or names that have been changed. If the "persona name" is a REGISTERED SCA name, simply marking "registered" in this column is fine. Similarly, if you KNOW that the persona name is already listed in the OP recognizeably, this field isn't strictly necessary. Please try to include this field if this person is 'new' to the OP (most often this will be for any Award of Arms), as this goes a LONG way to preventing time-consuming problems in the OP later on. Things that are useful in the 'reference' name field are:

A 'legal' name, or part of one
A previous persona name that may still be in the OP (when using a new persona name on this form)
A 'registered' SCA name
A widely recognized nickname (not always useful, but better than nothing)
